Clinical Significance:
Increased Levels:
- Hyperthyroidism: Painless thyroiditis, subacute thyroiditis, exophthalmic hyperthyroidism, thyroid medication intake, teratoma, malignant choriocarcinoma, pituitary thyrotropin-secreting tumor.
- Normal Thyroid Function: Increased TBG (thyroid-binding globulin) (familial), pregnancy, newborns, some liver cancers, acute hepatitis, acute intermittent porphyria, medications (steroids, contraceptives), positive anti-thyroxine antibodies in chronic thyroiditis, familial dysalbuminemic hyperthyroxinemia, excessive TBPA (thyroxine-binding prealbumin), transient hyperthyroxinemia (acute illness, oral cholecystography agents).
Decreased Levels:
- Hypothyroidism: Chronic thyroiditis, cretinism, iodine organification defects, pituitary hypothyroidism.
- Normal Thyroid Function: Decreased TBG (familial), nephrotic syndrome, dialysis treatment, hypoproteinemia, protein-losing enteropathy, liver cirrhosis, medications (testosterone, anabolic steroids, corticosteroids, salicylates, phenytoin, carbamazepine, heparin).
- Low T3 Syndrome: Severe illness, excessive T3 intake.
T4-Type Hyperthyroidism: Elevated T4 with normal T3.
